Parent Partnership Form

The Positive Partnership Communication Form is used to collect as much information from parents regarding about their child/ren as possible.

Below is a guide as to what information should be gathered in a conversation from the parent/guardian:

- key areas of interest

- any allergies and/or dietary requirements

- favourite foods, especially for babies it is important to know what foods they have tried and how these are being served

- dislikes and/or fears

- rest routine (i.e. how many/how long each nap is and timings if applicable)

- nappy changes and/or toileting (i.e. any reactions to specific brands of nappies, any nappy cream required etc)

- sunscreen (i.e. can we use our centre sunscreen or does the child have their own sunscreen)

- bottles (i.e. breast milk or formula, how many scoops/mls, times bottles are given)

- daily routine 

- comfort items (i.e. special blanket, dummy, teddy etc)

- developmental or educational goals (i.e. socialisation, walking etc)

- additional information (i.e. asthma, eczema, medical conditions, birth marks etc)

Once you submit this form it will appear in your Gmail/ZenDesk where you will need to copy and paste this information into the 'special considerations' section of a child's enrolment (the instructions are on the bottom of the email).

Instructions -  click on the child’s enrolment in Xplor, click on child account and paste the information into the ‘special considerations’ section. Please make sure to ALWAYS click save otherwise your information will not update to the enrolment.

This information can then be accessed by educators/teachers on playground. As changes occur with children’s information these can be updated accordingly. The best way to update this information would be to ask the parent/guardian to send an email with any new updates which will ensure that there is no miscommunication in the new updates.
